Mobile Clinic to Be Held at Caldwell First Nation Nov. 17, 18 & 19

Mobile Medical Support (MMS) is a community liaison team of healthcare professionals who help clients overcome barriers that prevent them from seeking healthcare and linking into community resources. The mobile clinic runs through the Windsor-Essex Ontario Health Team (WEOHT) and provides various supports including mental health and addiction and dental.

Its goal is to help navigate clients in need by providing health assessments, follow-up care, assistance with forms and referrals to care.

The Windsor-Essex Ontario Health Team (WEOHT) is made up of more than 45 healthcare organizations, community support services, hospitals, home and community care providers, lived experience partners, and primary care providers delivering services in urban and rural settings to over 425,000 patients.

Ontario Health Teams are a new model of integrated care delivery that will enable patients, families, communities, providers and system leaders to work together, innovate, and build on what is best in Ontario’s health care system.

The mobile clinic will be held at Caldwell First Nation, on Nov. 17, 18 and 19. No health card is required. The clinic is free for all ages.

The mobile clinic will offer these services:

  • Chronic disease support
  • COVID-19 and flu vaccines
  • Education and resources
  • Mental health support
  • Preventative care
  • Primary care check-up
  • Substance and addiction services
  • Support for adults, children, families, parents, seniors and teens
  • Testing
  • Wound and foot care

Dates & Times: 
Nov. 17 & 19 from 9 a.m. – 3:30 p.m.
Nov. 18 from 9 a.m.- 1p.m.
